General Award Guidelines:
•    Award can be recommended by any member but must be submitted by the agencies top ranking office.
•    Nomination form must be completed in the forms entirety.
•    Awards will be reviewed and approved by the Southwest Louisiana Mutual Association.
•    Awards will be presented at the Southwest Louisiana Mutual Association membership meeting by the President of the Southwest Louisiana Mutual Association.
•    All awards and photos will be posted to the Southwest Louisiana Mutual Association web page.
•    Awards can be for individuals, departments, committees and other groups.
